To make your application process run smoothly, we recommend gathering the following documents prior to downloading the 2013 CFC Application Form:

• IRS Tax Exemption Letter  (issued or updated within the past 5 years)
• IRS Form 990 (most recent version covering a period ending on or after 6/30/2011).
• Audited Financial Statement (dated on or after 6/30/2011) NOTE: The Form 990 and audited financial statements must cover the same fiscal period.).

IMPORTANT: If Audit and/or Form 990 are not available, you may still complete the application form and submit later.
